[VoIP] Patches

These patches, especially for the NZ/Oslo dial, should be submitted
so that they become part of the regular distribution and don't have
to be continually applied.  As long as they are under control of
options for the config files and work reliably, they should be
accepted.  If you submit them, please let me know the tracking
number, and I'll put in my oar in favor of them.
