[VoIP] Asterisk-Adtran dial rate

Max,

This morning I just did the exercise of watching the Asterisk-Adtran 
pulsing right after pulsing from a dial.

1. There is definitely a hesititaion between some digits. I think  about 
every 3rd digit is delayed.
2. Using larger numbers (but with the same ratio) in the make (52) and 
break  (78) constnts in zaptel.h does slow the pulsing down - and also 
makes the above more obvious.

The good news is that the settings I have now seem to also workaround 
the dialing errors. I think even  40 and 60 might do this. I will try 
them again.

Sorry getting to the above was so painful. Keeping time was my downfall 
when I tried piano!
