[VoIP] Strange extension dialing problem

I'm having a strange problem with my dialing plan as it relates to reaching
my PAX.  When I dial a PAX extension like 255-8841 (which is the busy tone)
from someone else's switch, it connects to the PAX correctly on extension
41.  However, when I dial that (or any other PAX extension) from my own
Asterisk PBX I get connected to the next higher extension than the one I
dialed.  If any CNET member dials 255-8841 they will get connected to the
correct extension.  When I dial from my switch I get connected to extension
42, and so on.  I've copied and pasted the console output so someone can
tell me what I'm missing here cause it's got me stumped.  The strange state
messages seem to be normal for any extension that's dialed to anything
that's hanging off my an FXO port.  The odd thing is that I used to be able
to get my PAX extensions correctly some time ago. I don't know exactly when
this odd behavior started.  Any ideas?
