[VoIP] ENUM Entry

Hi, folks.

Frank hadn't been put into the ENUM database because some of the
information on the form he submitted was missing.

Now, I'll take the rap for the form being less than intuitive, and maybe
some mouse-over help would be in order.  But, for anyone wanting to get
themselves added to the database, I really DO need ALL the information
on that form.  Otherwise, the lookup won't work.

Frank and I had a good exchange of emails, ending up with me setting him
up on an inbound username (or 'context') of cnet-inbound-calls.  So,
once that entry is put into place on his system, calls should complete.
